Documents for adoption from Ukraine
Here is a list of documents (Dossier) for adoption to be submitted to Adoption Center in Kiev, Ukraine, for your approval and registration as Prospective Parent(s):
1. Petition Letter requesting your registration as Prospective Parent(s).
2. Commitment Letter confirming your obligation to register a child in Ukrainian Consulate on the territory of the United States.
3. Home Study with favourable recommendation.
4. License of adoption agency that signed favourable determination.
5. Your preliminary approval by the US Citizenship and Immigration Services, Form I-171H
6. Your Passport(s).
7. Your Marriage Certificate (for couples).
8. Police clearance(s) issued by your residence State authorities.
9. Medical Conclusion(s), the specific Form required by Ukrainian Adoption Law is mandatory.
10. Your family income proof.
You may also want to issue a Power-Of-Attorney for your facilitator - Interpreter to represent you on the territory of Ukraine.
12. The Center for the Adoption of Children, within 20 working days, considers the application lodged by Ukrainian nationals residing outside the limits of Ukraine and by aliens. In case of positive decision, the Center registers the applicant as a candidate for the adopter and enters the information on him/her in the register of candidates for adopters. The date when candidates for adopters may come to obtain, from the centralized database, information on children who may be adopted is fixed upon agreement with the Center for the Adoption of Children.
Whenever the registration is refused, a written motivated reply shall be sent to the applicant together with the documents he/she has submitted.

The Independent Ukrainian adoption process starts from preparation documents in the United States. You should submit the required paperwork to National Adoption Center in Kiev, Ukraine.
Complete list of documents required for Independent Ukraine adoption, and the procedure for processing applications of aliens for international adoption in Ukraine are specified in the Resolution of the Cabinet Of Ministers Of Ukraine # 1377 that came into effect on January 1, 2004.

DOCUMENTS ARE IMPORTANT...
All documents must be separate documents; one cannot be part of another. The documents for adoption from Ukraine remain valid for one year from the date of issuance, unless the law of the issuing State provides otherwise.
All documents must be properly notarized and apostilled in the United States.
All documents for adoption from Ukraine shall be translated into Ukrainian. Translations must be certified and notarized by Ukrainian Notary. Translations of all documents should be performed professionally and comply with standard requirements. Translations may not include any mistakes or unacceptable terms.
Any dossier denied registration three times due to lack of proper documentation would be permanently rejected.
